Carl A. Minning, Jr., M.D.
Carl A. Minning, Jr., M. D. is a board certified ophthalmologist with special interest in cataract surgery, laser procedures, and Lasik. He has performed state of the art cataract surgery since the begining of his practice in 1983, and has been performing laser vision correction since 1995. In addition to being the founding member of Eye Surgery Associates of Zanesville, Inc., Dr. Minning is a Clinical Assistant Professor at the Ohio State University.
Dr. Minning received his undergraduate degree with a major in microbiology from the Ohio State University, with summa cum laude honors. He then graduated from the Ohio State University College of Medicine in 1979 where he was a member of the Alpha Omega Alpha Medical Honor Society. His internship was at Riverside Methodist Hospital and he then returned to the Ohio State University where he completed his residency in the Department of Ophthalmology from 1980-1983. During the academic year of 1982-1983, Dr. Minning served as the Chief Resident in the Department of Ophthalmology. Dr. Minning maintains membership or is a diplomate with the following groups and organizations:

- American Board of Ophthalmology
- American Academy of Ophthalmology
- American Society of Cataract and Refractive Surgeons
- American Medical Association
- Ohio State Medical Association
- Ohio Ophthalmological Society
He was also voted by his peers to "Best Doctors in America" 2003-2004, 2005-2006, and 2007-2008.
Dr. Minning lectures to medical and lay audiences on specific eye topics, especially the latest in cataract surgery and laser vision correction. He is a published author having multiple articles published in national peer reviewed medical journals, including the American Journal of Ophthalmology and the Archives of Ophthalmology. Dr. Minning's research interests include being one of the original investigators for bifocal intraocular lenses.
Dr. Minning has also been actively involved in organized medicine. He is on the Ohio State Medical Association Legislative Task Force, and is the Chairman of the Legislative Committee of the Ohio Ophthalmological Society. He has also been involved on the national level as a congressional advocate for both the American Society of Cataract and Refractive Surgery and the American Academy of Ophthalmology.
Dr. Minning has also been busy in various community activities. He is a past president of the Zanesville Art Center and is a lifetime trustee of the Zanesville Art Center. He has also served on the board of trustees of his local church, and has been involved with past fund raising for the Animal Shelter.

Shelly J. Temperley, M.D.
Shelly J. Temperley, M. D. is a board certified ophthalmologist who joined Eye Surgery Associates in 1991. Her special interests include cataract surgery and eyelid procedures, including the cosmetic and functional use of Botox.
She completed her undergraduate degree at Wayne State University in Detroit, Michigan with Phi Beta Kapa honors. She then attended the Wayne State University School of Medicine where she graduated in 1987 and was a member of the Alpha Omega Alpha Honor Society. She completed her residency at the Kresge Eye Institute at the Wayne State University in Detroit and became a fellow of the American Academy of Ophthalmology in 1995.

She has been the chair of several committees for the Genesis Healthcare System in Zanesville, and was also President of the Medical Staff at Genesis during 1999-2000.
Dr. Temperley has also been involved in clinical research related to ophthalmology, with several projects completed in conjunction with Pharmacotherapy Research Associates. Outside of the office, Dr. Temperley has been a very active member of Rotary, a member of the Zanesville-Muskingum County Board of Health, and she is active in Big Brothers/Big Sisters of Zanesville.
In Dr. Temperley’s spare time, she enjoys gardening, swimming, and other outdoor activities.

Ann E. Melick, M.D.
Ann E. Melick, M. D. is a native of the Zanesville area. She attended the Ohio State University where she graduated summa cum laude in 1986. She then graduated cum laude from the Ohio State University College of Medicine in 1991, where she was also a member of the Alpha Omega Alpha Honor Society.
She completed her residency in ophthalmology at the University of Minnesota in Minneapolis, where she won the Harry Plotke Award due to her excellent clinical skills associated with compassion and concern for her patients. Following her residency, Dr. Melick completed fellowship training in the subspecialty of glaucoma at Piedmont Hospital in Atlanta, Georgia. She then entered practice with Eye Surgery Associates in 1997, the same year that she became a fellow of the American Academy of Ophthalmology.
Dr. Melick is board certified and is a member of many professional organizations including:

She has lectured to lay and medical groups about various medical and surgical topics in the eye care field. Dr. Melick regularly attends national and international meetings, and even had a short stay in Columbia to learn the latest lasik techniques from world renowned ophthalmologist, Dr. Carmen Barraquer.
Dr. Melick stays very busy outside of the office with her husband and son. She also enjoys participating in equestrian events where she competes as a show jumper and occasional fox hunting.
